Managed Care Information Needed

Kings County became a managed care county in March 2011 which provides our residents the option for each family member to select either CalViva or Anthem Blue Cross as their provider. It is very important that you meet with a Health Care Options (HCO) representative to complete your paperwork when you apply, have been discontinued and reopened, or adding an individual to your medical assistance. Failure to meet with a HCO representative can delay your coverage up to 45 days or you can be placed into a plan that does not provide you the necessary doctors you may need. For more information on managed care please go to
http://www.healthcareoptions.dhcs.ca.gov/HCOCSP/Home/default.aspx. If you have questions or need to contact your managed care provider more information can be found at http://calvivahealth.org/ for CalViva or www.anthem.com for Anthem Blue Cross.
